You must call VDC Electronics Technical Support to recieve an RMA number prior to filling out this form or sending any unit back. Purchase date must be within 5 years of purchase. Return forms recieved without an RMA Number will not processed.

Please pack your BatteryMINDer carefully and include a copy of your original reciept. If your original reciept is not available, our technicians will determine the age of your unit when you call.
